Forms and Documents
This year our District is going to an online database to keep track of all of the forms and records that are required by the district and MHSAA. This means that all of the forms can be filled out electronically and don't have to be sent back and forth with your student. However, this is still an option if you are having trouble signing up or uploading some of the things that are needed. I know that not everyone has access to a scanner so we can still do that at the band hall if you don't have access. Just like CutTime, new programs online can be difficult to figure out so I will be posting the instructions here.
How to sign up:
This should take you to a page that looks like this:
Scroll down to the very bottom of the page and click on DO MY FORMS
Don't be alarmed by the AHSAA logo to the right. This program is in use in Alabama and Hinds County is one of the first in Mississippi to use it, you are at the right place!
Once you have clicked on DO MY Forms it will open a new window, scroll down and you should see a page that looks lilke this:
Click on the CREATE YOUR ACCOUNT & COMPLETE FORMS and that will take you to the Login Page
Click on Sign Up for Free and fill out the information and it will take you to a verification screen. The verification code will be sent to the email address or phone number that you signed up with.
Enter the code and proceed to the next page where you will see a list of ways to sign up, select parent at this screen:
Once you click on Parent it will ask you our school code. The code is FZBCBM.
It will then ask you if you want to continue as a parent or if you would like to reset. Continue as a parent and will take you to a screen where you can add your childs information. It will then ask for our school code again, this is the same code that was used in the previous step FZBCBM. You will then be taken to a create new athelete page with a list of options. Check Choir-Varsity (if your child is in 10th-12th). Check Freshmen Choir - JV (if your child is in 9th grade).
Scroll to the bottom of the page and hit save.
The next page will give you the option to add another child, view details, or submit paperwork. If you click on submit paperwork it will bring you to a page that shows you all of the work that needs to be completed, is in progress, or has not been completed.
Once you click on View Details or Submit Paperwork it will bring up the screen with all of the forms that are needed to be filled out.
Fill out each section and you are good to go!
