The following forms and information are needed to be eligible to participate in Athletics.The students participating in SPORTS will need the following required forms and documents:Required Forms - Parent consent, Concussion, Emergency Medical Authorization, Travel Form, and STAR Sportsmanship CourseRequired Documents - Valid Physical (No older than a Year from the completion of season) and Copy of Insurance CardRequired FormsSTAR Sportsmanship Course will be given to the Student-Athlete, by the Coaching StaffRequired Documentation that needs to be provided by the Parent/GuardianValid Physical - There will be an opportunity in May for Student Athletes to get a free physical from local providers. If your child was not able to attend the free physical, he/she can obtain one from their Doctor. Please make sure that the Doctor fills out an Athletic Participation Physical Form.The Coach will need Proof of Insurance.
