The following forms and information are needed to be eligible to participate in Athletics.

 

The students participating in SPORTS will need the following required forms and documents:

 

 Required Forms - Parent consent, Concussion, Emergency Medical Authorization, Travel Form, and STAR Sportsmanship Course

 Required Documents - Valid Physical (No older than a Year from the completion of season) and Copy of Insurance Card

STAR Sportsmanship Course will be given to the Student-Athlete, by the Coaching Staff

 

Required Documentation that needs to be provided by the Parent/Guardian 

Valid Physical - There will be an opportunity in May for Student Athletes to get a free physical from local providers.  If your child was not able to attend the free physical, he/she can obtain one from their Doctor.  Please make sure that the Doctor fills out an Athletic Participation Physical Form.

 

 

The Coach will need Proof of Insurance.  The copy will not be returned.  

 

 

 

All the above requirements must be met for your child to be eligible to participate in the Hinds County School District.
